  • Nursing Home Responsibilities Perform comprehensive physical therapy evaluations and develop individualized Plan of Care with measurable goals.
Implement evidence-based treatments including therapeutic exercises, gait and balance training, mobility and functional training. Monitor patient progress, adjust treatment plans as needed, and document outcomes in medical records per policy. Collaborate with an interdisciplinary team to support discharge planning and optimize functional independence. Ensure patient and staff safety, follow facility protocols, and uphold infection control standards. Requirements Active Physical Therapist license in Washington State or eligibility to obtain licensure. Doctor of Physical Therapy (DPT) degree or equivalent. Experience in a skilled nursing facility or geriatric rehabilitation setting preferred. Ability to work in a long-term care/SNF environment and contribute to an interdisciplinary team. Strong documentation, communication, and time-management skills; ability to manage a patient caseload within assigned hours.
