In this role, you will provide personalized rehabilitative care to patients in their homes, helping them regain mobility, manage pain, and improve overall quality of life. This position offers an opportunity to work closely with patients in a home health setting, delivering essential therapy services that make a meaningful difference in their daily lives. Duties Assess patients' physical conditions and develop individualized treatment plans tailored to their specific needs and goals. Deliver hands-on therapy interventions, including exercises, manual therapy, and modalities, to promote recovery and functional independence. Monitor patient progress regularly and adjust treatment plans accordingly to ensure optimal outcomes. Educate patients and their families on therapeutic exercises, injury prevention, and health management strategies. Collaborate with physicians, caregivers, and other healthcare professionals to coordinate care plans and ensure comprehensive patient support. Maintain accurate documentation of patient assessments, treatment sessions, progress notes, and discharge summaries in compliance with healthcare regulations. Adhere to all safety protocols and standards for home health care environments to ensure patient and staff safety. Stay current with advancements in physical therapy techniques and best practices through ongoing education. Skills Valid state licensure as a Physical Therapist. Proven experience working in a home health or similar outpatient setting preferred. Strong interpersonal skills with the ability to connect compassionately with patients of diverse backgrounds. Excellent assessment and clinical reasoning abilities. Ability to develop effective treatment plans tailored to individual patient needs. Good organizational skills for managing multiple patients and documentation requirements. Flexibility and adaptability to work independently in a home environment while maintaining professional standards. Knowledge of safety procedures specific to home health care settings.
