Job Description

We are seeking a Full-Time Physical Therapist (PT) to support our skilled nursing therapy team on an as-needed basis. These roles are ideal for licensed clinicians looking for flexible scheduling while delivering high-quality rehabilitation services in a skilled nursing environment.
Key Responsibilities PT Responsibilities:
Perform evaluations and develop individualized physical therapy treatment plans Supervise PTA treatment when applicable Collaborate with interdisciplinary teams to support resident goals and discharge planning Provide skilled therapy services to short-term rehab and long-term care residents Complete accurate and timely documentation in accordance with Medicare and facility guidelines Participate in care planning and interdisciplinary communication as needed Qualifications Active PT in WI.
