Position Summary We are seeking a compassionate and skilled Physical Therapist (PT) to join our growing home health team in Rock Springs, WY. In this role, you'll provide one-on-one, patient-centered care in the comfort of patients' homes—helping them regain independence and improve quality of life. ✨ New graduates are strongly encouraged to apply! What You'll Do Evaluate and develop individualized care plans for home health patients Deliver evidence-based physical therapy interventions Educate patients and caregivers on treatment plans and progress Collaborate with an interdisciplinary team to ensure optimal patient outcomes Maintain accurate and timely clinical documentation Adapt care based on patient progress and needs Why Home Health?
