00 per hour, based on experience Kern River Transitional Care is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. The PTA will work under the direction of a licensed Physical Therapist to help residents improve mobility, restore function, and achieve their highest level of independence. Key Responsibilities Provide physical therapy treatments according to the plan of care established by the Physical Therapist Assist residents with therapeutic exercises, mobility training, transfers, balance, and gait activities Observe and document residents' progress, responses, and changes in condition Communicate resident updates and concerns to the supervising Physical Therapist Educate residents, families, and caregivers on exercises and safety techniques Maintain accurate and timely clinical documentation Follow all facility policies and applicable state and federal regulations Collaborate with nursing, rehabilitation, and interdisciplinary team members Promote a safe, supportive, and resident-centered environment Qualifications Graduate of an accredited Physical Therapist Assistant program Current California PTA license in good standing Current CPR certification Previous skilled nursing or long-term care experience preferred Strong communication, documentation, and interpersonal skills Compassionate, dependable, and committed to providing excellent resident care Benefits Competitive pay Flexible PRN scheduling Supportive and collaborative rehabilitation team Continuing training and professional growth opportunities Positive and rewarding work environment
