Responsibilities Assist in developing, implementing, and modifying patient care plans under the supervision of licensed physical therapists. Provide hands-on therapy techniques such as therapeutic exercises, manual therapy, trigger point therapy, and modalities tailored to individual patient needs. Monitor and document patient progress accurately in accordance with clinical documentation standards and electronic health records (EHR) systems. Support discharge planning processes by preparing patients for transition to home or community care settings. Collaborate effectively with healthcare professionals including physicians, nurses, occupational therapists, and social workers to optimize treatment outcomes. Work with diverse patient populations including geriatric patients, pediatrics, neurology cases, orthopedics, and outpatient rehabilitation clients. Educate patients and their families on treatment procedures, home exercises, and safety measures to promote ongoing recovery outside clinical settings. Requirements Valid certification or licensure as a Physical Therapist Assistant in accordance with state regulations. Prior experience in acute care, inpatient hospital settings, outpatient clinics, or outpatient rehabilitation centers is highly preferred. Demonstrated knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques relevant to patient care. Experience working with geriatric patients, pediatrics, neurology cases, orthopedics or home health environments is advantageous. Familiarity with clinical documentation standards and proficiency in electronic health records (EHR) or electronic medical records (EMR) systems such as Epic or Cerner. Strong communication skills for effective client interaction and interdisciplinary collaboration. Ability to work independently and as part of a multidisciplinary team within fast-paced inpatient or outpatient environments. Knowledge of discharge planning processes and experience supporting patient rehabilitation goals in various settings including hospital or outpatient clinics.
