A Day in the Life (Your Responsibilities) Implement treatment plans established by the supervising Physical Therapist. Assist residents with therapeutic exercises, gait training, transfer training, balance activities, and mobility interventions. Monitor patient progress and communicate changes in condition to the supervising therapist. Educate residents and caregivers on exercises, safety precautions, and proper use of adaptive equipment. Maintain accurate and timely documentation in accordance with facility, state, and federal regulations. Collaborate with interdisciplinary team members, including Physical Therapists, Occupational Therapists, Speech Therapists, Nursing Staff, Social Workers, and Physicians. Participate in care planning meetings and quality improvement initiatives. Ensure compliance with all Skilled Nursing Facility policies and rehabilitation standards. What It Takes to Succeed (Qualifications) License Current and unrestricted Physical Therapist Assistant (PTA) license in the State of California (Required). Education Graduate of an accredited Physical Therapist Assistant program. Experience Skilled Nursing Facility (SNF), rehabilitation, long-term care, or geriatric experience preferred. New graduates with strong clinical rotations are encouraged to apply. Certification Current Basic Life Support (BLS) certification. Skills Strong patient care and treatment implementation skills. Excellent communication and interpersonal abilities. Ability to work independently and collaboratively within an interdisciplinary team. Strong organizational and documentation skills.
