We are a physical therapy staffing agency partnering with home health agencies to provide high-quality therapy services in patients' homes. We are seeking reliable, compassionate, and experienced Physical Therapy Assistants who are comfortable working independently in the home health setting. Responsibilities Provide physical therapy treatments in patients' homes under the supervision of a licensed Physical Therapist. Follow the established Plan of Care and treatment goals. Accurately document each visit in the client's EMR within required timeframes. Communicate patient progress, concerns, and changes in condition to the supervising Physical Therapist. Educate patients and caregivers on exercises, safety, mobility, and fall prevention. Maintain compliance with Medicare, state, and agency documentation standards. Coordinate with the home health agency and scheduling team regarding visit completion and availability. Qualifications Current California Physical Therapy Assistant (PTA) license in good standing. Minimum 1 year of Home Health experience preferred. Current CPR certification. Valid driver's license, reliable transportation, and auto insurance. Strong documentation and communication skills. Ability to work independently and manage a flexible schedule. Experience with electronic medical records (EMR) preferred.
