Responsibilities Assist licensed physical therapists in developing and implementing individualized care plans tailored to each patient's needs. Perform therapeutic exercises, manual therapy techniques such as trigger point therapy, and assist with modalities to promote healing and mobility. Conduct comprehensive clinical assessments and document patient progress accurately in accordance with clinical documentation standards. Operate medical equipment safely and effectively during treatment sessions, ensuring proper usage and maintenance. Maintain detailed medical documentation, including progress notes, treatment records, and discharge planning documentation within electronic health records (EHR). Collaborate seamlessly with healthcare professionals, caregivers, and family members to ensure cohesive patient care and effective communication. Support discharge planning by evaluating patient progress and providing recommendations for ongoing care or referrals as needed. Skills Proven experience in home health or skilled nursing facility settings with a strong understanding of outpatient, inpatient, and acute care environments. Expertise in physiotherapy techniques including manual therapy, orthopedics, geriatrics, pediatrics, and sports rehabilitation. Knowledge of medical terminology, physiology, anatomy, and clinical assessment documentation standards. Skilled in the operation of medical equipment used during physical therapy treatments. Proficiency in electronic health records (EHR) management for accurate medical documentation and reporting. Excellent communication skills for engaging with clients and caregivers effectively while providing clear instructions and compassionate support. Ability to adapt treatment approaches based on client needs across diverse populations including geriatrics and pediatrics.
