At Prava , we're building a different kind of rehabilitation model for senior living—one where clinicians are truly embedded in the community, supported by technology, and given the time and space to focus on what matters most: the person in front of them. We're looking for a Lead Physical Therapist Assistant who is passionate about working with older adults living with dementia and understands that meaningful therapy isn't always about a perfect exercise or a bigger number. It's about helping someone move with confidence and participate in the life they have today. What you'll do Become a familiar, trusted presence within a 60-apartment memory care community. Provide physical therapy interventions that are meaningful, functional, and individualized under the direction of a licensed Physical Therapist. Help residents maintain mobility, strength, balance, safety, and independence. Adapt interventions and the environment to meet residents where they are. Partner with caregivers and community staff to carry therapeutic strategies into everyday routines. Look beyond the diagnosis to understand the person, their habits, preferences, abilities, and goals. Collaborate closely with the Prava clinical team while becoming part of the community you serve. Use Prava's technology to reduce administrative burden and give you more time for clinical care.
