Physical Therapy Assistant in Santa Clara/San Jose, CA We are seeking a qualified Physical Therapy Assistant with a minimum of 1 year professional experience for a contract position with CVHCare. The ideal candidate will have home health experience and be assigned to Territory Area #11 (Santa Clara, Sunnyvale, Cupertino, Milpitas, Mt. View, Campbell, Los Altos, Morgan Hill, Mt. Hamilton, Saratoga, Los Gatos, San Martin) and Area #12 San Jose. Option 1: Guaranteed Hours - The candidate must have productivity of at least 30 points per week (points described below). The guarantee is for 36 hours, and the traveler must complete 30 points in the week. The candidate must be willing to travel outside the area or work weekends if required to maintain weekly visit expectations. If the traveler refuses patients, the guarantee does not apply. The guarantee does not apply for the first and last week of assignment or for weeks in which there are holidays or the traveler has time off. Option 2: Non-Guaranteed Hours - Weekend coverage may be required. To secure a full caseload, the traveler may be required to go outside the service area occasionally. The candidate can refuse patients, and there are no guaranteed hours. The candidate must be willing to see Covid+ and Covid Pui patients. Payment is made on a per-visit basis. Credentials required:

Current CA PTA License, BLS, Drivers License, Auto ins, Auto Reg, TB (annual), MMR, Varicella, HepB, TDap (10 years), Flu (annual), Drug screen, Physical. Start Date and End Date must be identical. All Requested Time Off (RTO) must be disclosed at the time of submission. If the agency or traveler does not include all RTO in the submission form and adds it post confirmation, the agency will be at risk of being removed from this client's vendor set. No exceptions!
