JOB DESCRIPTION

Physical Therapist will evaluate mobility, strength and current level of function while setting goals to improve balance, walking, transfers, as well as patient education. Treatment and exercise programs, physical therapist will also make recommendations on equipment, provide training on the proper use of the equipment, and train family members or caregivers to help maintain exercise programs.

REQUIREMENTS

Graduate of a Physical Therapy program with a Bachelor of Science degree in Physical Therapy, Masters or Doctorate degree preferred. Current California Physical Therapy license. A minimum of one (1) year of experience Home health experience preferred. Current CPR Certification. Valid CA driver's license with automobile insurance coverage with proof of good driving record. Reliable transportation Must be able to perform physical therapy interventions in the home environment. Interact effectively with the public and team members. Ability to comply with accreditation standards, regulations, and agency policies/procedures. Complete an on-site functional assessment, review, and revision of the Plan of Care, and an assessment of the utilization of community resources.
