Job Description

South Pacific Rehab Services is seeking a dedicated Physical Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will provide direct patient care under the supervision of a Physical Therapist in skilled nursing and post-acute settings. Responsibilities include implementing treatment plans, documenting progress, and collaborating with an interdisciplinary team to promote functional recovery and quality of life. Responsibilities Provide direct PTA treatments under PT supervision in skilled nursing/post-acute settings Carry out individualized treatment plans to improve mobility, function, and safety Accurately document treatments, progress, and resident response in EMR systems Educate patients, families, and caregivers on exercises and functional strategies Collaborate with PTs, nurses, and facility staff to support resident-centered goals Monitor patient status, report changes promptly, and uphold ethical standards of care Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Modalities application (heat, cold, e-stim, ultrasound) Treatment documentation in EMRCare plan implementation Patient and family education Safety and fall-prevention techniques Interdisciplinary team collaboration Time management in skilled nursing setting
