In this role, you will work closely with licensed physical therapists to implement treatment plans and assist patients in their recovery process. Your expertise will contribute to improving patients' mobility, strength, and overall quality of life in various settings, including outpatient clinics, skilled nursing facilities, and acute care environments. Responsibilities Assist physical therapists in implementing individualized treatment plans based on patient assessments. Provide therapeutic exercises and modalities to enhance patient recovery. Monitor patient progress and report any changes or concerns to the supervising therapist. Educate patients and their families on exercises, care plans, and home programs. Maintain accurate documentation of patient treatments and progress notes as required. Participate in discharge planning to ensure a smooth transition for patients post-treatment. Collaborate with interdisciplinary teams to optimize patient care. Skills Strong knowledge of anatomy and physiology relevant to physical therapy practices. Experience or familiarity with acute care settings, orthopedics, geriatrics, outpatient services, and skilled nursing facilities. Ability to develop and implement effective care plans tailored to individual patient needs. Proficient in discharge planning processes to facilitate successful patient outcomes. Excellent communication skills for educating patients and collaborating with healthcare teams. Strong organizational skills with attention to detail in documentation and reporting.
