Job Description

Solaris HealthCare is seeking a Certified Physical Therapy Assistant to provide compassionate, hands-on rehabilitation services to seniors in our skilled nursing and long-term care community. Working under the direction of a licensed Physical Therapist, you will carry out treatment plans, lead therapeutic exercises, assist with gait and balance training, and support safe mobility and transfers. You'll document progress, educate residents and families, and collaborate with a team that values respect, integrity, and resident-centered care, with growth opportunities across our Florida facilities. Responsibilities Provide therapeutic exercises, gait training, and mobility activities under supervision of a licensed Physical Therapist Assist residents with transfers, balance, and functional activities to promote safety and independence Document treatments, resident responses, and progress accurately in the EMR system Follow individualized care plans and contribute observations to support updates by the PTEducate residents and families on exercises, safety techniques, and use of assistive devices Maintain a safe, clean treatment environment and adhere to infection control protocols Collaborate with nursing, therapy, and interdisciplinary team members to support resident goals Comply with state practice guidelines, facility policies, and ethical standards of care Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility assistance Modalities application (heat, cold, e-stim, etc.) Treatment documentation in EMRCare plan implementation Patient and family education Range of motion techniques Safety and fall prevention practices Interdisciplinary team collaboration
