Job Description

Palmetto Subacute Care Center seeks a compassionate Physical Therapist Assistant to provide hands-on therapy in our skilled nursing and rehab setting. Under the direction of a licensed PT, you will deliver individualized treatment plans focused on mobility, strength, and functional independence for post-acute, subacute, and long-term residents. You'll document progress, educate patients and families, and collaborate with an interdisciplinary team in a respectful, resident-centered environment that supports professional growth and clinical excellence. Responsibilities Provide direct patient care under the supervision of a licensed Physical Therapist following individualized care plans Assist patients with therapeutic exercises, gait training, balance, and functional mobility activities Accurately document treatments, patient progress, and responses in the medical record Collaborate with PTs, nurses, and interdisciplinary team to support resident-centered rehab goals Educate patients and families on home exercise programs and safe mobility techniques Maintain a safe, clean therapy environment and ensure proper use of equipment Participate in care plan meetings and contribute to discharge planning Uphold regulatory, ethical, and facility standards for skilled nursing and rehab care Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Treatment documentation in EMRSkilled nursing/rehab clinical skills Patient and family education Transfer and wheelchair safety Vital signs monitoring Interdisciplinary team collaboration Knowledge of PT treatment modalities
