Job Summary:
We are seeking dedicated and compassionate PTA to provide high-quality home health services to patients in their place of residence. The ideal candidates will deliver skilled therapy services focused on improving patient mobility, independence, safety, and overall functional outcomes. Therapists and assistants will collaborate closely with the interdisciplinary care team to ensure optimal patient care.
Key Responsibilities:
Deliver therapy services in accordance with the established Plan of Care Implement therapeutic interventions to improve mobility, strength, balance, coordination, and activities of daily living Assist patients in achieving maximum independence and functional outcomes Accurately document patient evaluations, progress notes, and treatment outcomes Communicate effectively with physicians, supervising therapists, and interdisciplinary team members Educate patients and caregivers on exercises, home programs, adaptive equipment, and safety techniques Maintain compliance with all state, federal, Medicare, and agency regulations
Qualifications:
Active Florida state license as a PTA Minimum of 1 year of home health experience preferred Strong clinical assessment, documentation, and communication skills Ability to work independently and manage a flexible schedule Reliable transportation and valid driver's license
Preferred Skills:
Experience with electronic medical records (EMR) systems Knowledge of Medicare guidelines and home health documentation standards Bilingual (English/Spanish) is a plus
