In this role, you will work closely with licensed physical therapists to provide high-quality patient care in various settings, including clinics and home health environments. Your primary focus will be on assisting patients in their rehabilitation journey, implementing care plans, and ensuring optimal recovery outcomes. Responsibilities Assist physical therapists in developing and implementing individualized treatment plans for patients. Provide direct patient care, including therapeutic exercises, modalities, and manual therapy techniques. Monitor patient progress and report any changes or concerns to the supervising physical therapist. Educate patients and their families on treatment protocols, exercises, and home health care strategies. Maintain accurate documentation of patient treatments and progress notes in compliance with regulatory standards. Participate in discharge planning to ensure a smooth transition for patients leaving the facility. Collaborate with interdisciplinary teams to optimize patient care and outcomes. Uphold safety standards and infection control procedures within the clinic or home health setting. Qualifications Associate degree in Physical Therapist Assisting from an accredited program. Current state licensure or certification as a Physical Therapist Assistant. Knowledge of anatomy, physiology, and principles of physical therapy practices. Experience in acute care, geriatrics, or home health settings is preferred but not required. Strong communication skills to effectively interact with patients, families, and healthcare team members. Ability to develop and implement care plans tailored to individual patient needs. Commitment to providing compassionate patient care while maintaining professionalism at all times.
