The ideal candidate will have a strong foundation in physical therapy practices and a passion for delivering exceptional, patient-centered care across a variety of clinical settings. As an integral member of the rehabilitation team, the Physical Therapist Assistant will work under the direction of licensed physical therapists to implement individualized treatment plans, support patient assessments, monitor progress, and collaborate with patients, families, caregivers, and other healthcare professionals. Medical Specialty Home Health Key Responsibilities Assist licensed physical therapists with developing and implementing individualized treatment plans based on each patient's condition, goals, and functional needs. Provide therapeutic exercises, manual therapy techniques, including trigger point therapy, and appropriate physical therapy modalities to support recovery and improve mobility. Monitor, evaluate, and accurately document patient progress in accordance with clinical documentation requirements and healthcare standards. Safely operate, inspect, and maintain physical therapy equipment and treatment-related medical devices. Communicate effectively with patients, caregivers, physicians, therapists, and other healthcare professionals to ensure coordinated and consistent patient care. Educate patients and their families on prescribed exercises, therapeutic techniques, post-treatment care, home programs, and injury-prevention strategies. Maintain accurate and timely electronic health records (EHR), including treatment notes, clinical documentation, progress updates, and other required records. Participate in care planning and discharge preparation, particularly for patients transitioning from acute care or skilled nursing facilities to home-based care. Provide rehabilitation support across a variety of patient populations and settings, including home health, inpatient, outpatient, skilled nursing, geriatrics, pediatrics, orthopedics, and acute care, as needed. Maintain a professional, compassionate, and patient-focused approach while promoting safety, independence, mobility, and overall quality of life. Qualifications & Requirements Current and valid licensure or certification as a Physical Therapist Assistant (PTA) from an accredited program, as required by applicable state regulations. Demonstrated experience implementing physical therapy treatment plans in inpatient, outpatient, home health, or other clinical settings. Strong knowledge of anatomy, physiology, medical terminology, therapeutic exercise, and clinical documentation standards. Proficiency in manual therapy techniques, including trigger point therapy, as well as appropriate physical therapy modalities. Experience using physical therapy equipment and electronic health record (EHR) systems. Excellent communication and interpersonal skills, with the ability to work effectively with patients and caregivers from diverse backgrounds and age groups, including geriatric and pediatric populations. Understanding of discharge planning and transitions of care from acute care and skilled nursing facilities; home health experience is highly desirable. Ability to work effectively as part of a multidisciplinary healthcare team while maintaining professionalism, accountability, and patient confidentiality.
