What You'll Do Provide skilled physical therapy treatments in patients' homes under the direction of a licensed Physical Therapist. Implement individualized treatment plans to improve mobility, strength, balance, safety, and overall function. Accurately document patient visits and progress in agency EMR systems. Communicate effectively with supervising therapists, physicians, nurses, and caregivers. Educate patients and families on home exercise programs and fall prevention strategies. Maintain compliance with Medicare and agency regulations. Qualifications Current Florida Physical Therapist Assistant license in good standing. Home health experience preferred but not required. Strong communication and time-management skills. Reliable transportation and valid driver's license. Ability to work independently while maintaining excellent patient care.
