Job Description

Powerback Rehabilitation is seeking a Physical Therapy Assistant to deliver patient-centered care across post-acute, outpatient, and home-based settings. In this role, you'll provide hands-on treatment under the direction of a Physical Therapist, helping patients restore mobility, strength, and independence after illness, injury, or surgery. You'll implement individualized treatment plans, document progress, and educate patients and families on safe movement and home exercises. Working in a collaborative, clinically driven environment, you'll be part of an interdisciplinary team focused on measurable outcomes, continuous learning, and supporting patients in getting back to their best selves. Responsibilities Provide hands-on physical therapy interventions under the direction of a licensed Physical Therapist Assist with patient assessments, documenting progress and reporting changes in condition Implement individualized treatment plans to improve mobility, strength, and functional independence Instruct patients and families on exercises, safety, and use of assistive devices Collaborate with an interdisciplinary rehab team across post-acute, outpatient, and home-based settings Maintain accurate, timely clinical documentation and comply with regulatory standards Prepare and organize therapy equipment and treatment areas for safe, efficient care Support a patient-centered environment focused on measurable functional outcomes Participate in ongoing clinical training and quality improvement initiatives Educate patients on home exercise programs and strategies to prevent functional decline Required Skills Therapeutic exercise implementation Gait training and balance intervention Functional mobility training Patient transfer and safe handling techniques Use of therapeutic modalities (e.g., heat, cold, e-stim, ultrasound) as allowed by state law Clinical documentation in EMR systems Patient and family education Knowledge of post-acute and geriatric rehab principles Care planning under PT supervision Interdisciplinary team collaboration
