Job Description

CenterWell Home Health seeks a compassionate Home Health Physical Therapist Assistant to deliver in-home therapeutic care. Under the direction of a Physical Therapist, you'll implement treatment plans, assist with mobility and strength training, educate patients and caregivers, and accurately document progress. You'll collaborate closely with an interdisciplinary team to promote safety, independence, and optimal outcomes. Responsibilities Provide in-home physical therapy treatments under the supervision of a Physical Therapist Implement and progress individualized care plans to improve mobility, strength, and functional independence Assist with gait training, transfer techniques, and fall prevention strategies Educate patients and caregivers on home exercise programs, safety, and use of assistive devices Monitor, record, and report patient status, response to treatment, and changes in condition Accurately document all care in EMR in compliance with regulatory and agency standards Collaborate with PTs, nurses, and other clinicians to coordinate comprehensive care Adhere to infection control, safety, and quality standards in the home setting Travel between patient homes, managing daily schedule and visit routes effectively Required Skills Therapeutic exercise implementation Gait training and balance interventions Home safety assessment Patient and caregiver education Vital signs monitoring Electronic medical record (EMR) documentation Pain management techniques within PTA scope Therapeutic modalities application (e.g., heat, cold, e-stim per policy) Infection control and universal precautions Time management and route planning
