Job Description

Work Schedule:
100%, day/eve, Monday-Friday / 8:00am-5:00pm / Rotating weekends / Rotating holidays. Your primary work location will be 2550 Charles St Rockford IL. You may be eligible for a $5,000 sign on bonus Additional components of compensation may include:
  • Evening, night, and weekend shift differential
  • Overtime

  • Completion of an accredited program approved by the American Physical Therapy Association. Required Work Experience Licenses & Certifications
  • Current license from the State of Illinois or in the process of acquiring licensure. Required
  • Valid State driver's license and proof of liability insurance. Required
  • Bi-Annual CPR certification. Required
  • An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment.
An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided.
