  • Part-time, PRN Job Overview We are seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our home care team in Lake County, IN.
Previous Home visit experience a plus, but not mandatory. In this role, you will provide essential rehabilitative services to patients in their homes, supporting recovery and improving quality of life. The ideal candidate will have a strong background in patient care, physiology, and various therapy techniques. This position offers an opportunity to work with diverse patient populations within the home health setting. The PTA will collaborate closely with supervising therapists to develop and implement individualized care plans and discharge strategies. Responsibilities Deliver therapeutic interventions tailored to each patient's needs. Assist in developing and modifying personalized care plans in collaboration with supervising therapists. Provide patient care focused on improving mobility, strength, and functional independence in the home health setting. Educate patients and caregivers on exercises, safety measures, and discharge planning to promote ongoing recovery. Document patient progress accurately in compliance with healthcare regulations and facility policies. Support discharge planning processes by assessing patient needs and coordinating with multidisciplinary teams. Monitor patient responses to treatments and adjust interventions as necessary to ensure safety and effectiveness. Maintain a safe environment by adhering to infection control protocols and safety guidelines during all patient interactions. Qualifications Valid license or certification as a Physical Therapist Assistant. Proven experience in physiotherapy practices such as outpatient clinics, inpatient settings, skilled nursing facilities, or home health environments. Knowledge of anatomy, physiology, medical terminology. Experience working with diverse populations including geriatrics, orthopedics, and patients recovering from acute care or surgeries. Strong understanding of care plans and discharge procedures within healthcare settings. Excellent communication skills for effective collaboration with healthcare teams and patient education. Ability to work independently in home health settings while maintaining professional standards of care. Prior experience with home care setting a plus, but not strictly necessary.
