This team member would have the opportunity to work alongside a veteran Physical Therapy team within an incredible, 130+ unit, state-of-the art senior living community that opened in 2024. Responsibilities Develop and (or) implement individualized physical therapy treatment plans aligned with patient goals and clinical assessments. Physical Therapists will perform thorough evaluations of patients' physical conditions, including mobility, strength, flexibility, and functional limitations. Physical Therapist Assistants will carry out the robust treatment plan to meet outcomes desired for improvement in skill deficits of the client. Operate and maintain medical equipment safely and effectively during therapy sessions for all clients. Document all clinical assessments, treatment interventions, progress notes, and discharge planning accurately following medical documentation standards. Collaborate closely with all parities involved in the care of the client which could include (but is not limited to) the client themselves, their family, community leadership, the Carefirst Rehab clinical team, Carefirst Rehab leadership, liaisons, and other professionals working to care for our clients. Educate clients and caregivers on exercises, injury prevention strategies, and post-treatment care to promote ongoing recovery at home or in facility settings. Qualifications Valid licensure or certification as a Physical Therapist or Physical Therapist Assistant in accordance with state regulations. Proven experience in outpatient clinics, inpatient units, skilled nursing facilities, or home health environments is preferred. Strong knowledge of physiology, anatomy, orthopedics, geriatrics, pediatrics, and medical terminology relevant to physical therapy practice. Proficiency in clinical assessment documentation and electronic health records (EHR) management. Ability to communicate effectively with patients of diverse backgrounds and coordinate seamlessly with healthcare teams. Prior experience with discharge planning and working within acute care or outpatient settings is highly desirable.
