Physical Therapist Assistant (Part-Time)
Location:
Valparaiso, Indiana Join the One Home Health Team One Home Health is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to provide high-quality care to patients in their homes throughout the Valparaiso area. This part-time opportunity offers flexibility, meaningful one-on-one patient interactions, and the chance to make a lasting impact on individuals' health and independence. What You'll Do As a Physical Therapist Assistant, you will work under the direction of a licensed Physical Therapist to deliver exceptional home health services and support positive patient outcomes. Key Responsibilities Provide physical therapy treatments and patient education in the home according to the physician-approved Plan of Care. Complete patient visits and documentation accurately, timely, and in compliance with agency standards. Monitor patient progress and promptly communicate any changes in condition, treatment response, or scheduling needs to the supervising therapist or nurse. Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team to coordinate patient care. Participate in care conferences and contribute to patient care planning as appropriate. Adhere to all safety protocols, infection prevention procedures, and proper equipment-use guidelines. Protect patient privacy and maintain strict confidentiality in accordance with HIPAA and agency policies. Ensure compliance with agency policies and all applicable state, federal, and professional regulations. Participate in ongoing education, staff meetings, and quality improvement initiatives. Perform other duties as assigned to support patient care and organizational goals. What You'll Bring Required Qualifications Current and unrestricted Physical Therapist Assistant license in the state of Indiana. Graduate of an accredited Physical Therapist Assistant program approved by the American Physical Therapy Association (APTA). Minimum of six (6) months of experience as a Physical Therapist Assistant. Strong verbal, written, and interpersonal communication skills. Reliable transportation with a valid driver's license, current automobile insurance, and a satisfactory driving record.
