Job Description

Job Summary We are seeking a dedicated and energetic Home Health Physical Therapist Assistant (PTA) to join our team on a PRN basis. In this vital role, you will work closely with licensed physical therapists to deliver exceptional patient care in home health settings. Your expertise will help patients regain mobility, reduce pain, and improve their overall quality of life. This position offers flexibility and the opportunity to make a meaningful difference in patients' lives by providing personalized, compassionate therapy services across diverse environments. Duties Assist licensed physical therapists in developing and implementing individualized care plans tailored to each patient's needs, including those in geriatrics, pediatrics, orthopedics, and post-acute care. Deliver hands-on therapy interventions such as therapeutic exercises, manual therapy techniques, and trigger point therapy to promote healing and mobility. Monitor patient progress regularly, document outcomes accurately, and communicate changes or concerns to supervising therapists promptly. Educate patients and their caregivers on proper techniques for exercises, mobility aids, and discharge planning to support ongoing recovery at home. Maintain detailed records of treatments provided, patient responses, and any modifications needed to optimize care plans. Collaborate with interdisciplinary teams including skilled nursing facilities, outpatient clinics, and acute care providers to ensure seamless patient transitions and continuity of care. Adhere strictly to safety protocols and medical terminology standards while working in inpatient or outpatient settings. Qualifications Proven experience as a Physical Therapist Assistant with a strong background in physiotherapy practices including anatomy knowledge and physiology fundamentals. Hands-on experience with trigger point therapy, manual therapy techniques, and discharge planning processes. Familiarity with home health environments as well as inpatient settings such as skilled nursing facilities or outpatient clinics. Knowledge of medical terminology relevant to orthopedics, geriatrics, pediatrics, and acute care cases. Ability to work independently while maintaining clear communication with supervising licensed therapists and healthcare teams. Strong understanding of care plans tailored for diverse populations including geriatrics and pediatric patients. Prior experience in outpatient or clinic settings involving physiotherapy treatments is highly desirable. Join us in delivering compassionate, expert care that transforms lives!
