Job Description

Quad C Physical Therapy is looking to hire a Physical Therapist for our new location in Leesville, LA but also commute to our Many location 2 days a week. After June 1st the position will be fully at Leesville. At Quad C we believe a team approach with high quality care is most important when it comes to helping our patients achieve their highest level of function.

M-TH 8am-5pm, F 8am-12pm

Essential Job Functions/Responsibilities:
1. Provides patient care that is ETHICAL, in accordance to Physical Therapy Rules of Louisiana Board of Physical Therapy Examiners and APTA Standards of Practice. 2. Performs a comprehensive evaluation/re-evaluation of patient in regards to physical therapy needs upon referral in a timely manner as determined by Quad C Physical Therapy's policy. 3. Establish Short/Long Term Goals determined by evaluation in conjunction with patient, physician, caregiver, and additional disciplines involved with patient care. 4. Provides interventions that require clinical skills and knowledge of a Physical Therapist consistent c established goals. Interventions will be within Physical Therapist's scope of practice in which he/she is competent and or received advanced continuing education. 5. Provides documentation in a timely manner for inclusion in patients medical record per Quad C Physical Therapy's policy. 6. Maintains ongoing effective communication with patient, physical, caregiver, and additional disciplines and staff involved with patient care. 7. Provides ongoing education and instruction through home exercise program to the patient and caregiver.
Minimum Requirements:
Education
  • Graduated from an accredited Physical Therapy Program License
  • Current Louisiana Physical Therapy License Experience
  • New Grads and Experienced Therapist applicable

$50,000.00
  • $70,000.
