Job Description

Job Summary We are seeking a dedicated and compassionate Physical Therapy Assistant (PTA) to join our Lagniappe Homecare team in Natchitoches, La. and covering surrounding communities. The PTA will work closely with a licensed physical therapist to deliver high-quality rehabilitative care to diverse patient populations in their homes. This involves implementing individualized treatment plans, assisting with clinical assessments, and ensuring accurate medical documentation.
Key Responsibilities:
Assist licensed physical therapists in developing and implementing comprehensive treatment plans tailored to patient needs Monitor patient progress and document clinical assessments Explain procedures to clients while addressing concerns and motivating clients to follow care plans. Work with healthcare professionals to coordinate discharge planning and ensure continuity of care Maintain detailed medical documentation in compliance with regulatory requirements
Requirements:
Certification as a Physical Therapy Assistant from an accredited program; valid state licensure or registration required Prior experience working in skilled nursing facilities or home health environments preferred Experience operating medical equipment used in physical therapy treatments Proficiency managing electronic health records (EHR) Strong communication skills Demonstrated understanding of discharge planning processes and client care coordination
