Physical Therapy Assistant - Home Care Holyoke VNA & Hospice - 3.2 Holyoke, MA Job Details Per diem 1 hour ago Qualifications Associate's degree in physical therapist assistant Licensed Physical Therapist Assistant Associate's degree Full Job Description The Holyoke VNA & Hospice Life Care is looking for a Per Diem Physical Therapist Assistant for Home Care Experience. With prior approval and evaluation by the Physical Therapist, assist the Physical Therapist when the Physical Therapist is performing tests, evaluations and complex treatments. Accept responsibility for the patient and the patient's environment during treatment. Provide treatments as delegated by the Physical Therapist including but not limited to, application of heat/cold/light/water/sound/massage, performance of exercises, trains the patient/caregiver in exercises/ambulation/activities of daily living, positioning (including body alignment and body mechanics) and uses assistive devices in care. The performance of these treatments must be in accordance with accepted standards of care and policies. Document patient care and participate in record keeping. Observe record and report to the Physical Therapist the conditions, reactions and responses related to his/her duties. Responsible to maintain his/her quality and safety of care through continuing education. Cares for assistive devices and HVNA equipment as per manufacturer's instructions and policy. Work in a cooperative manner to provide safe care. Refers all inquiries regarding patient status to a Physical Therapist or Clinical Manager and performing other duties as necessary or assigned.
EDUCATION
Graduate of a two (2) year associate Degree Program approved by the American Physical Therapist Association.
EXPERIENCE
One (1) to three (3) years experience in a health care setting.

Therapy Schedule:
Per Diem, day shift, 8am-4:30pm
