Job Description
Renewal Rehab is seeking a compassionate Physical Therapist Assistant to join our Healthcare & Medical Services team supporting seniors in skilled nursing and post-acute care. In this on-site role, you will carry out individualized treatment plans under the direction of a Physical Therapist, focusing on mobility, strength, balance, and functional independence. You'll document outcomes in EMR, collaborate with an interdisciplinary team, and educate patients and families. Responsibilities Implement treatment plans under the direction of a Physical Therapist for senior patients in skilled nursing and post-acute care settings. Provide therapeutic exercises, gait training, and functional mobility interventions to improve strength, balance, and independence. Accurately document treatments, progress, and functional outcomes in the electronic medical record in a timely manner. Educate patients, families, and caregivers on home exercise programs, safety, and functional strategies. Collaborate with an interdisciplinary team to support patient-centered, evidence-based care and quality outcomes. Assist with screenings, reassessments, and discharge planning as directed by the supervising Physical Therapist. Maintain compliance with all regulatory, company, and professional standards, including infection control and safety. Participate in mentorship, in-service training, and professional development offered by Renewal Rehab.
