Home Health Physical Therapy Assistant:
Provide rehabilitative physical therapy under the supervision of a licensed Physical Therapist, following each patient's individualized Plan of Care. Assist patients with exercises and mobility techniques to improve strength, balance, coordination, and endurance. Document patient status, observations, and care provided promptly and thoroughly. Monitor progress and collaborate with the Physical Therapist to adjust the plan as needed. Demonstrate exercises or therapy techniques for caregivers, home health aides, or family members who will assist the patient. Encourage patients and promote independence and safety in the home. Deliver care in alignment with federal and state guidelines and professional standards. Build trusting relationships with patients and their families to support consistent, high-quality care. About You Qualifications -
What You'll Bring:
Graduate of an accredited two-year Physical Therapy Assistant program. Licensed, certified, or registered PTA as required by applicable state law. In states without specific licensure requirements, must be a graduate of an accredited PTA program. Current CPR certification required. Strong interpersonal and documentation skills. Valid driver's license, auto insurance, and reliable transportation.
