What You'll Do:

Implement care plans crafted by a licensed PT to improve strength, safety, and function Provide home-based therapy including mobility support, ROM, balance training, and gait work Track and report progress using streamlined EMR software Teach exercises, safety tips, and self-care strategies to patients and caregivers Work closely with PTs, nurses, and the broader care team to ensure top-tier outcomes

Qualifications:

Current Minnesota Physical Therapy Assistant license Reliable transportation, insurance, and valid driver's license A warm communicator with strong motivation to deliver patient-centered care Home health experience preferred, with thorough onboarding provided
