Strong Full-Time Earning Potential Schedule:
Full-Time + Weekend Rotation How To Stand Out Interested applicants are encouraged to email Nikky Vogelgesang at nicole.vogelgesang@hhcare.net with a copy of their resume and a brief overview of their background, why home health interests them, and what they enjoy most about helping patients regain mobility and independence. Why More PTAs Are Choosing Home Health Burnout is real. Many Physical Therapist Assistants find themselves running from patient to patient, juggling unrealistic productivity expectations, and feeling like they no longer have enough time to truly connect with the people they serve. Home care changes that. At Legacy Home Care and Home Health Care, Inc., you'll spend your days providing one-on-one treatment where patients are often most motivated to recover—their own homes. You'll witness victories that matter: the first time a patient safely walks to the mailbox again, returns to church, or regains enough strength to remain independent. Instead of being one provider among many, you'll become a trusted part of your patients' recovery journey. Why Clinicians Stay Here Strong compensation and excellent earning potential. Full benefits package including medical, dental, disability coverage, paid time off, and 401(k) matching. Manage your own territory and daily schedule. One-on-one patient care with meaningful relationships. Stable referral base and consistent full-time volume. Collaborative partnership with experienced Physical Therapists, nurses, and interdisciplinary team members. Supportive leaders who understand therapy and believe clinicians deserve autonomy and flexibility. Additional Perks We're Especially Excited About Free access to our MedBridge Education Hub for continuing education and professional development. Licensure reimbursement when your license is due for renewal. Annual scrub reimbursement. Mileage reimbursement between patient visits. Cell phone reimbursement paid every paycheck. What Makes This Role Different As a PTA, you'll help patients recover from surgery, illness, injury, and hospitalization while allowing them to remain in the comfort of home. Under the direction of a Physical Therapist, you'll implement treatment plans that improve mobility, strength, balance, and safety.
Home care offers something unique:
you don't just see how patients perform in a gym or clinic. You see how they live. That perspective allows you to provide therapy that is practical, individualized, and deeply meaningful. Responsibilities Provide physical therapy visits in patient homes. Implement treatment plans established by the supervising Physical Therapist. Assist patients with exercises, gait training, balance activities, and strengthening programs. Educate patients and caregivers on home exercise programs and safety strategies. Monitor patient progress and communicate updates to the supervising PT. Complete timely and accurate EMR documentation. Collaborate with nurses, therapists, physicians, and other healthcare professionals. Qualifications Current Minnesota Physical Therapist Assistant (PTA) license. Valid driver's license and reliable transportation. Strong communication and patient engagement skills. Ability to work independently in the field. Home health experience is welcome but not required. Why Legacy Home Care & Home Health Care, Inc.? For decades, our organizations have helped Minnesota families remain safe and independent at home. We believe great outcomes begin with great clinicians, and great clinicians deserve flexibility, support, and the opportunity to focus on what they do best. If you're looking for a career where relationships matter, autonomy is valued, and every visit has purpose , we'd love to connect.
