As a Physical Therapy Assistant with Sovereign Healthcare Group, you will contribute to and implement individualized plans of care under the supervision of licensed therapists. You will also work as an integral part of the inter-professional team to provide education to patients, families, and staff and assist in all levels of care planning and transitions.

The Physical Therapy Assistant will report directly to the Director of Rehabilitation Services.

None Responsibilities include:
- Contributes to physical therapist's effectiveness by identifying patient care issues; providing information and commentary; recommending options and courses of action; implementing physical therapy directives.
- Maintain the expected productivity level that has been discussed and is expected for your position at your particular facility/facilities.
- Documents patient care services by charting in patient and department records.
- Performs therapeutic procedures by administering manual exercises; instructing, encouraging, and assisting patients in performing physical activities, such as non-manual exercises, ambulatory functional activities, and daily-living activities; and in using supportive devices, such as crutches, canes, and prostheses.
- Instructs and counsels patients by describing therapeutic regimens; giving normal growth and development information; promoting wellness and health maintenance.
- Provides continuity of care by developing and implementing patient management plans. Maintains safe and clean working environment by complying with procedures, rules, and regulations; adhering to infection-control policies and protocols.
- Maintains production objectives by noting current treatment trends; monitoring actual physical therapy services rendered to production objectives.
- Maintains legal and accreditation compliance by adhering to policy positions concerning federal, state, and local regulations, and JACHO standards; anticipating emerging issues.
- Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
- Enhances physical therapy and hospital reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments. Perform other duties as assigned and required to provide quality care to patients/residents.
- Understand reimbursement by various payers and allocates therapy resources efficiently in accordance with patient/resident goals and outcomes related to discharge destination.
- Demonstrate a knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third party reimbursement guidelines.
- Support the company's commitment to Compassionate Care; projects a professional image to foster confidence in the Rehab Program.
- Perform duties in an independent manner and recognizes situations where direct supervision is needed; willingly helping others. Qualifications
- Degree from an accredited Physical Therapy Assistant program.
- Licensed by the State in the designated geographic area in his/her respective therapy profession.
- Demonstrates the ability to perform the following tasks without posing a threat to the health or safety of him/herself, co-workers, caregivers, or patient; lift, turn, move position, and transport the patient into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
- Ability to complete the following activities safely, and at times, for prolonged periods:
Standing:
Maintaining an upright position on the feet, especially for prolonged periods of time.
Pushing:
Applying pressure and exerting force against an object for the purpose of moving it forward or away from the source of the force - push or pull force up to 35 lbs.
Lifting:
Raising an object from a lower to a higher position, or moving an object laterally, from position to position
- Required to lift up to 50 lbs.

Stooping:
