Job Description

Physical Therapist Assistant needed at Avenir at Maple Grove in Louisiana, MO! We are seeking a compassionate and motivated PRN Physical Therapist Assistant (PTA) at Avenir at Maple Grove in Louisiana, MO to join our rehabilitation team in a Skilled Nursing Facility setting. The PTA will work under the direction and supervision of a licensed Physical Therapist to provide high-quality rehabilitative care to our residents, helping them restore mobility, improve functional independence, and enhance overall quality of life. Responsibilities Implement treatment plans developed by the Physical Therapist Provide therapeutic exercises, gait training, balance training, transfer training, and mobility interventions Utilize modalities and therapeutic techniques as appropriate Monitor and document patient progress accurately and timely Communicate resident status and changes to supervising PT and interdisciplinary team Educate patients and caregivers on home exercise programs and safety strategies Ensure compliance with federal, state, and facility regulations Maintain productivity standards and complete required documentation Qualifications Graduate of an accredited Physical Therapist Assistant program Current state PTA license (in good standing) CPR certification (or willingness to obtain) Previous experience in a Skilled Nursing Facility preferred Strong clinical, documentation, and communication skills Ability to work collaboratively within an interdisciplinary team Compassionate, patient-centered approach to care This position operates in a Skilled Nursing Facility providing short-term rehabilitation and long-term care services. The role requires standing, walking, lifting, transferring patients, and assisting with mobility throughout the workday.
