This role plays an essential part in helping residents achieve their highest practical level of function while maintaining dignity and quality of life. Essential Responsibilities Carry out treatment plans established by the supervising Physical Therapist. Provide therapeutic exercises, gait training, balance activities, transfer training, and functional mobility interventions. Accurately document treatment sessions, resident progress, and responses in accordance with facility, state, federal, and payer requirements. Observe, report, and communicate changes in residents' conditions to the supervising therapist. Collaborate with nursing, physicians, and other interdisciplinary team members to promote optimal resident outcomes. Educate residents and family members on therapy techniques, safety, and home exercise programs as appropriate. Maintain a clean, safe, and organized therapy environment. Comply with all Medicare, Medicaid, HIPAA, OSHA, and facility policies and procedures. Qualifications Current Missouri Physical Therapist Assistant (PTA) license in good standing. Graduate of an accredited Physical Therapist Assistant program. Current CPR certification (or ability to obtain upon hire). Skilled nursing, rehabilitation, or long-term care experience preferred, but new graduates are encouraged to apply. Strong communication, organizational, and documentation skills. Ability to work collaboratively within an interdisciplinary team. Genuine compassion and commitment to providing exceptional resident-centered care.
