About the Role We are seeking compassionate, reliable, and skilled Physical Therapist Assistants (PTAs) to join our home health team. You will work under the direction and supervision of a licensed Physical Therapist to implement personalized treatment plans that help patients recover from injury, surgery, or illness and regain independence in their daily lives. Key Responsibilities Provide skilled physical therapy interventions in patients' homes as outlined in the PT's plan of care Assist patients with therapeutic exercises, functional mobility training, gait training, transfer training, and use of assistive devices Perform manual therapy techniques, modalities (ultrasound, e-stim, etc.), and therapeutic activities Educate patients and caregivers on home exercise programs, safety, and fall prevention Document progress notes accurately and timely in our EMR system (experience with Homecare Homebase or similar preferred) Communicate effectively with the supervising PT, case managers, and other members of the interdisciplinary team Manage a productive caseload while maintaining compliance with Medicare and state regulations Qualifications Graduate of an accredited Physical Therapist Assistant program Current PTA license in [State] (or eligible for immediate licensure) Valid driver's license, reliable transportation, and auto insurance Current CPR/BLS certification Minimum 1 year of clinical experience preferred (new grads with strong home-health interest will be considered) Home health experience is a plus but not required•we provide training and mentorship Excellent time-management and documentation skills Compassionate, patient-centered approach with the ability to work independently What We Offer Competitive pay Flexible scheduling (full-time, part-time, or per diem) Mileage reimbursement Health, dental, and vision insurance (for full-time employees) 401(k) with company match Paid time off and holidays Continuing education reimbursement Supportive clinical team, weekly case conferences, and direct access to supervising PTs Laptop/tablet and supplies provided If you're a motivated PTA who loves building relationships with patients in their own homes and wants the flexibility and autonomy that home health offers, we'd love to meet you!
