I. JOB SUMMARY
The Home Health Physical Therapy Assistant (PTA) at Big Sky Home Health and Hospice , a proud affiliate of The Pennant Group , provides physical therapy services to patients in their homes under the supervision of a licensed Physical Therapist (PT). This role supports patient independence, functional mobility, and quality of life through high‑quality, compassionate, and patient‑centered care delivered in accordance with Montana state law, federal regulations, and agency policies. The PTA is an integral member of the interdisciplinary care team and is expected to uphold Big Sky Home Health and Hospice's commitment to clinical excellence, compliance, accountability, and service.
II. DUTIES & RESPONSIBILITIES
Provide skilled physical therapy services as delegated by and under the supervision of a Montana‑licensed Physical Therapist, following the established plan of care. Implement therapeutic interventions including exercises, gait training, balance and coordination activities, transfer training, functional mobility training, and pain‑management techniques within the PTA scope of practice. Observe, assess, and document patient responses to treatment and communicate progress, concerns, or changes in condition promptly to the supervising PT. Educate patients and caregivers on home exercise programs, safety techniques, fall prevention strategies, and proper use of assistive devices. Assist patients in improving strength, endurance, balance, mobility, and functional independence within their home environment. Identify environmental risks within patient homes and take appropriate steps to ensure patient and personal safety during all visits. Complete accurate, timely, and thorough documentation in the electronic medical record in compliance with Big Sky Home Health and Hospice policies, Medicare Conditions of Participation, and Montana regulations. Collaborate with the interdisciplinary team including nurses, therapists, physicians, hospice staff (as applicable), and care coordinators to ensure coordinated, patient‑centered care. Maintain strict patient confidentiality and comply with HIPAA and privacy requirements at all times. Participate in team meetings, case conferences, staff education, and quality improvement initiatives. Adhere to infection control standards, including the appropriate use of personal protective equipment (PPE). Maintain current licensure, certifications, and competency requirements. Represent Big Sky Home Health and Hospice and The Pennant Group professionally, ethically, and respectfully in all interactions with patients, families, and community partners.

III. PRODUCTIVITY & PERFORMANCE EXPECTATIONS
Meet established productivity standards as defined by Big Sky Home Health and Hospice, including visit expectations, quality outcomes, and patient satisfaction. Complete all required documentation accurately and within agency‑defined timeframes (generally within 24-72 hours of the visit). Maintain consistent attendance and timely completion of scheduled patient visits. Participate in performance improvement and quality assurance activities as required.
IV. JOB REQUIREMENTS
Graduate of an accredited Physical Therapy Assistant program. Current, active, and unrestricted Physical Therapy Assistant license in the State of Montana . Ability to practice under supervision of a Montana‑licensed Physical Therapist in accordance with the Montana Board of Physical Therapy Examiners. Current CPR/BLS certification. Home health experience preferred; will train motivated candidates with strong clinical foundations. Successful completion of background check, drug screening, and health screening requirements. Valid Montana driver's license, reliable transportation, and current auto insurance. Ability to work independently, manage time effectively, and adapt to diverse home environments, including rural settings. Strong interpersonal, verbal, and written communication skills. Basic proficiency with electronic medical records and mobile documentation technology.

VI. PHYSICAL REQUIREMENTS
The physical demands described below are representative of those required to perform essential job functions: Frequent standing, walking, bending, kneeling, squatting, and reaching Ability to assist with patient transfers and mobility Ability to lift, push, or pull up to 50 pounds with or without assistance Fine motor skills required for therapeutic interventions and documentation Visual and auditory acuity necessary for patient assessment and safety Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
