Typically Monday-Friday, with flexibility based on FTE and patient care needs. Job Summary We are seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our home health team. In this role, you will deliver rehabilitative care in patients' homes under the direction of a licensed Physical Therapist, helping patients improve mobility, function, and overall quality of life. Key Responsibilities Provide physical therapy services in patients' homes in accordance with established plans of care and under PT supervision Implement rehabilitative interventions following guidance from the supervising Physical Therapist Communicate promptly with the supervising PT regarding: Changes in patient condition or need for medical attention Issues with treatment plans or patient tolerance Plateaus or progress toward goals Changes in homebound status Hospitalizations or transitions of care Any concerns affecting patient safety or care quality Maintain accurate, timely documentation of all patient care activities Educate patients, families, and staff on safe mobility, transfers, and body mechanics Participate in interdisciplinary team meetings and provide patient updates Ensure patient confidentiality in accordance with agency policies Perform additional duties as assigned Minimum Qualifications Graduate of an accredited Physical Therapist Assistant (PTA) program Current PTA license in the State of Montana Minimum of 2 years of PTA experience preferred Valid driver's license , reliable transportation, and current auto insurance Strong communication and organizational skills Ability to work independently in a home health setting Apply Today!
