  • PRN, where you will play a vital role in delivering exceptional patient care across diverse settings.
This flexible position offers the opportunity to work in the home health setting. As a key member of the rehabilitation team, you will support physical therapists by implementing treatment plans, assisting with therapy sessions, and ensuring patients receive personalized care that promotes recovery and independence. Duties Assist licensed physical therapists in developing and executing individualized care plans for patients across various settings such as inpatient units, outpatient clinics, or home health visits Implement therapeutic interventions including trigger point therapy and other manual techniques to reduce pain and improve mobility Monitor patient progress during therapy sessions, document responses, and communicate updates to the supervising therapist Support discharge planning by preparing patients for transition from active treatment to home or other care environments Educate patients and their families on exercises, mobility strategies, and injury prevention techniques tailored to specific needs such as orthopedics, geriatrics, pediatrics or post-surgical recovery Maintain accurate records of treatments provided, patient responses, and progress notes in compliance with healthcare regulations Ensure safety protocols are followed during all therapy activities while fostering a motivating and compassionate environment Qualifications Valid licensure as a Physical Therapy Assistant (PTA) in accordance with state regulations Proven experience working in skilled nursing facilities, outpatient clinics, or inpatient settings with exposure to orthopedics, geriatrics, pediatrics or acute care environments Strong knowledge of anatomy, physiology, medical terminology and physiotherapy techniques including trigger point therapy Familiarity with care planning processes and discharge procedures for diverse patient populations Excellent patient care skills with the ability to motivate and support individuals through their rehabilitation journey Ability to adapt treatment strategies based on patient needs and progress assessments Prior experience working in home health or outpatient clinics is preferred but not required Join us to be part of a dedicated team committed to enhancing recovery outcomes through expert therapy interventions.
