Setting:

Long-term care / assisted living environment Responsibilities Implement and progress PT plans for active patients under supervision, focusing on functional mobility, strength, and balance. Assist with therapeutic exercises, gait training, transfers, and activities of daily living training. Prepare and position patients for therapy sessions; monitor patient response and report changes to the supervising PT. Document treatment provided, progress notes, and any deviations from the plan of care in the medical record. Maintain a safe, clean, and organized therapy area; ensure equipment is in good working order. Collaborate with an interdisciplinary team to support discharge planning and patient goals.
