Physical Therapist Assistant Needed - Help Patients Thrive Right Where They Live. Retention bonus of $7,500 payable in quarterly installments during your first year of employment. At HCS, we're dedicated to providing exceptional, compassionate home care to individuals recovering from surgery, managing chronic conditions, or improving mobility. We're looking for a skilled and caring Physical Therapist Assistant to join our team and help patients achieve their rehabilitation goals. This position is full time with hourly pay in the Peterborough/Keene area.
Responsibilities:
Implement patient care plans developed and supervised by a Registered Physical Therapist, delivering hands-on therapeutic services in patients' homes and community settings. Provide skilled therapeutic interventions to maintain, improve, or restore patient function across neuromusculoskeletal, pulmonary, and cardiovascular conditions. Collaborate with the care team including supervising therapists, nurses, physicians, and other providers to ensure coordinated, patient-centered care. Educate patients and families on therapeutic procedures, home exercise programs, and strategies to support recovery and independence. Document clinical progress accurately and in accordance with HCS standards and regulatory requirements. Monitor and ensure patient safety during all therapy sessions, relaying significant changes in patient status to the supervising PT in a timely manner. Why Join Us?

Requirements:
Graduate of a state-approved Physical Therapist Assistant program One year of experience under appropriate supervision; or a combination of education and experience demonstrating the necessary competencies for the position Current New Hampshire license to practice as a Physical Therapist Assistant Ability to utilize a portable electronic field device and manage ePHI remotely in accordance with HCS policies Valid driver's license and access to a vehicle meeting NH registration and auto liability insurance requirements Home Healthcare, Hospice and Community Services (HCS) is a non-profit organization providing comprehensive services in southwestern New Hampshire. Clinical services include visiting nurses, physical and occupational therapies, social work and home health aides. Hospice at HCS offers end of life care and grief support for patients and family members wherever they call home, including assisted living and nursing homes. HCS is unique in the wide range of community services that are offered to the community, including Healthy Starts family services, Meals on Wheels and senior dining, City Express and Friendly Bus transportation, and Nurse Is In foot care clinics. HCS is Medicare certified and licensed by the State of New Hampshire.
