Job Description
Eagle Rock Post Acute Care at Cedar Grove seeks a compassionate Physical Therapist Assistant to support high-quality rehab services for seniors in our skilled nursing and rehabilitation facility. Working under the direction of a licensed PT, you will deliver individualized treatment interventions, assist with therapeutic exercises and mobility training, and promote residents' safety, function, and independence. You will document care in the EMR, collaborate closely with an interdisciplinary team, communicate with families, and contribute to a resident-first culture focused on dignity, respect, and clinical excellence while growing your skills in post-acute and long-term care.
