Job Description
Eagle Rock Post Acute Care at Cedar Grove is seeking a Licensed Physical Therapist Assistant to join our compassionate, resident-first team. In this on-site role, you will provide hands-on therapeutic interventions under the supervision of a Physical Therapist for post-acute, short-term rehab, and long-term care residents. Responsibilities include implementing individualized treatment plans, documenting progress, educating residents and families, and collaborating closely with an interdisciplinary team.
