Physical Therapist (PT) / Physical Therapist Assistant (PTA) Homeside Rehab Join Our Growing Home Health Team! Homeside Rehab is seeking compassionate and dedicated Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to provide individualized, evidence-based physical therapy services to patients in their homes throughout New York City and Yonkers, NY . If you're passionate about helping patients improve mobility, strength, balance, coordination, and functional independence while enjoying a flexible schedule and supportive clinical team, we'd love to hear from you! Responsibilities Perform comprehensive physical therapy evaluations (PTs) and develop individualized treatment plans based on each patient's needs and goals. Provide evidence-based physical therapy interventions to improve strength, balance, coordination, mobility, gait, endurance, and functional independence. Deliver skilled therapy services in patients' homes while promoting improved function and quality of life. Complete accurate, timely, and compliant electronic documentation. Collaborate with physicians, caregivers, and the interdisciplinary healthcare team to coordinate patient care. Educate patients and caregivers on home exercise programs, mobility strategies, and therapeutic techniques. Communicate effectively with office staff regarding scheduling, patient updates, and clinical needs. Qualifications Active New York State Physical Therapist (PT) or Physical Therapist Assistant (PTA) License (Required) Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or Skilled Nursing Facility (SNF) preferred Excellent clinical assessment and communication skills Ability to work independently in a home health setting Basic computer proficiency for electronic documentation Valid New York State driver's license and reliable transportation
