Job Description

Spring Creek Nursing and Rehabilitation is currently seeking an experienced, responsible, and motivated PRN Physical Therapy Assistant. Therapists should possess passion for geriatric population, must contain a license in the state of Ohio, be able to evaluate and treat independently without direct supervision, and posses the desire to perform functional therapeutic activities to assist residents improve in their ADLs. Spring Creek provides specialized care services for patients requiring extended medical care and rehabilitation. Those with catastrophic or acute illnesses and injuries can work towards recovery. We also offer both short-term rehabilitation and long-term skilled nursing care, along with a full respiratory unit that provides ventilator care and all respiratory needs. Responsibilities and Duties Experience with long-term care and/or geriatric care is preferred but not limited to. Well versed with documentation writing Provides appropriate treatment, preventative measures an adaptive equipment to prevent decline in resident function. Weekly progress notes, recertification and discharge documentations. Collaborates with all disciplines to provide best patient centered treatment and care. Exceptional patient service skills and ability to demonstrate clinical excellence Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times. Perform all other duties assigned by Director of Rehabilitation Compliance with confidentiality, HIPPA and Federal Residents Rights.
