Interim HealthCare is seeking a compassionate Physical Therapy Assistant to deliver high-quality therapy services in patients' homes. In this role, you'll support licensed Physical Therapists by assisting with treatment plans, therapeutic exercises, and mobility training tailored to each patient's needs. You'll document progress, educate patients and families, and collaborate closely with our interdisciplinary team. Responsibilities Provide hands-on support to Physical Therapists in delivering care plans for home health patients Assist patients with therapeutic exercises, mobility training, and use of assistive devices Monitor, document, and report patient progress, responses, and concerns to supervising PT and clinical team Educate patients and families on home exercise programs and safety techniques Adhere to care plans, infection-control standards, and regulatory guidelines in the home setting Collaborate with nurses, therapists, and caregivers to ensure coordinated, patient-centered care Maintain accurate, timely clinical documentation in accordance with agency policies Travel to patient homes and manage daily schedule efficiently and professionally Required Skills Therapeutic exercise implementation Gait and balance training Home safety assessment Patient and family education Clinical documentation and EMR use Knowledge of musculoskeletal and neurological conditions Vital signs monitoring Transfer and mobility techniques Infection control practices Time management and scheduling in home health
