PHYSICAL THERAPIST ASSISTANT
Job Description Provides physical therapy services under the direction of the physical therapist and according to the physician's plan of care. 1.
Qualifications:
Graduate of an accredited physical therapy assistant program with either an Associate degree in science or applied science which is approved by American Physical Therapy Association, Inc. Current Certification or License as required by State Law. 2.
Responsible to:
to the Physical Therapist. 3.
Responsibilities and Duties:
Provides direct physical therapy according to directions of physical therapist and in accordance with the physician's plan of care. Treats patient/client to relieve pain, develop or restore function and maintain maximum performance. Directs and aids patients/clients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training. Utilizes physical agents, including, but not limited to heat, cold, water, light, electricity, ultrasound and massage. Observes and reports to the physical therapist and the Organization personnel the patient's/ client's reaction to treatment and any changes in patient's/client's condition. Instruct patient's/client's in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices. Maintains necessary records; clinical notes and conference notes which will be incorporated into the patient's/client's clinical record in prescribed time frames. Participates in Organization Home Care Team Members meetings and Organization In-service Education Program. Performs other duties as assigned. 4.
Submission of Paperwork:
The clinical and progress notes of each discipline maybe faxed to the agency within 5 days of completion of each patient visit. Original copies of such notes shall be submitted and included with the current patient's medical record within 7 days of the visit completion." These forms are: 1) Progress Notes/Visit Notes 2) Weekly Route Sheet 3) Patient's Visit Schedule - Monthly 5. See section VI for other contract requirements under Policies for Contract Services. 6.
Physical Requirements:
Must be able to work as scheduled and report to work on time. Can stand or sit up to six hours per day. Able to lift up to 20 pounds on a frequent basis. Able to push wheelchairs, turn patients, help patients walk, lift patients and help patients sit up four to five hours per day. Able to walk, stand, bend, climb and crawl four to five hours per day. Normal vision required, including depth perception, color perception, close inspection, peripheral vision. Glasses or contact lenses may be used to insure normal (20-20) vision. Normal hearing required, including ability to hear normal speech, hear on Telephone, and hear faint sounds. Able to talk in person and on the telephone. Able to deal with stressful situations, including patient death, family grie and trauma. Able to deal with patients, patient care givers, and employees of residential care facilities six to eight hours per day. Able to operate an automobile two to four hours per day. 7.
Working Condition:
Work generally performed in patients' homes or residential care facilities. Work requires employees to drive where work is performed and employees will be outdoors, in or out of an automobile, one to two hours per day.
Pay:
$50.00 - $60.00 per hour
