Opens a new window. Job Summary Provides Physical Therapy services within scope of practice and under the direction of a registered Physical Therapist and according to plan of care. Qualifications Associates degree from an accredited therapy program required. Current California certification meeting APTA requirements required. BLS required within 60 days of hire. Minimum of one year of PTA experience required. Previous home health experience preferred. PTA scope of practice. Proficient in computer use. Able to work under minimal direction and multi-task effectively. Communicates effectively with patients/families and all levels of the organization. Current drivers license and proof of insurance required. Bilingual (Spanish/English) preferred. Professional Duties Maintain member confidentiality at all times (HIPAA). Provides direct planned patient therapy services (within scope of practice) under the direction of a Registered Physical Therapist and according to agency policies and plan of care. Implements the plan of care as initiated by the Registered Physical Therapist and/or practitioner which establishes goals and incorporates therapeutic, preventative and rehabilitative therapy services as appropriate. Instructs patient and family in care and use of wheelchairs, braces, crutches, canes, prosthetic or orthotic devices, and/or any other assistive device(s) as appropriate. Provides health teaching to patient, and/or caregiver regarding treatment and plan of care. Education will utilize terms and teaching methods that are customized to the learning needs of the patient and/or caregiver. Communicates any changes in the patient condition or plan of care to the RPT, MD, team members and patient/caregiver. Provides accurate and timely documentation consistent with the plan of care; This may include preparing clinical and progress notes as appropriate. Services. The Clinical Practice Pharmacist provides medication therapy management for patients as defined under medical staff approved protocols. This individual supports and contributes to process improvement within the PHARxM clinics and department and participate in the training end mentorship of Pharmacy Students and Pharmacy Residents. Professional Duties Follow medical staff approved protocols in management of patients defined under PHARxM department. Provide comprehensive medication therapy consults to patients referred to PHARxM. Collaborate with the interdisciplinary team to ensure optimal patient care coordination. Document patient interactions and consultation's in the patient's electronic medical/health record (EMR/EHR) in accordance with established departmental policies, procedures and timeframes. Conduct critical patient monitoring and comprehensive chart reviews to identify, prevent, and mitigate drug-related problems, including adverse drug reactions, drug interactions, medication omissions, and inappropriate use. Conduct thorough medication histories, assess patient compliance/progress, and suggest modifications to achieve desired treatment outcomes. Responsible for coordinating all requests for services not provided in the SNF (i.e. specialist consults, radiology etc.) while maintaining Traxit updates in a timely manner. Attend weekly SNF ICT meetings and provides constructive input towards patient goals along with a timely & appropriate discharge plan of care. Completes SNF skilled and custodial ICT recap report and sends report to SNF Team. Ensures that NOMNC/denial letters are initiated timely and according to Health Plan and regulatory agencies specifics. Contacts admitting and/or consulting physicians directly, when information is necessary to clarify the medical plan of care, level of care, and/or discharge challenges. Keeps Medical Director and/or Designee apprised of SNF activity and needs.
